Registration Information

Changing the World ... One kid at a time.

Community Kids Activities Club - Registration Information

Registration for our 2012 Summer Program will begin on April 23rd and continue until May 4th.

 

Please, be sure to bring ALL of the following items when you come to register!

  • Immunization Form
  • 2-3 names, addresses and good phone numbers for people who can pick up your children
  • Child's social security number
  • Child's doctor's name, address and phone number
  • Health Insurance Information (Carrier and Policy Number)
  • Families First Case Number or Food Stamp Number (if applicable)
  • Names of all household members
  • Proof of income for fee assessment. This includes:
    • Earnings from work per month for each household member
    • Child Support per month (if applicable)
    • Pensions, retirement and/or social security benefits per month (if applicable)
    • Any other monthly income
